- Bilateral LASIK with VISX S2 Excimer Laser in October 1998 for hyperopia
- Unable to drive at night since surgery. Ghosting of images with glare and halos. Very poor near vision even with bifocals
- Pre op refraction OD + 2.25 + 1.25 x 100 20/20 OS + 2.25 + 1.25 x 045 20/20
- Post op refraction OD Plano + 1.00 x 077 20/25 OS + 0.25 + 0.50 x 053 20/20
